Graduate Student Award Checklist
NACTA GRADUATE STUDENT AWARD CHECKLIST
(Must be included with nomination)
** Note: All recipients are expected to attend the NACTA Awards Conference and Banquet. NACTA does not cover expenses: registration, travel, lodging, meals, etc.
- Nomination packet with label (send four copies) postmarked by March 1st. NOTE - No copies will be returned, so don't send the original. When putting the nomination packet together, please use a soft cover or soft notebook (not a hardcover notebook).
- A photograph and short biography of each award recipient will be included in the awards program at the annual conference. Submit a biography not to exceed 250 words in WordPerfect or Word format, along with a good picture in the form of a JPG or GIF file (the larger/better quality, the better your picture will be). The biography and picture should be sent via e-mail to the Chair of the Teacher Recognition Committee.
- Cover page - Include: addresses, phone numbers & e-mail information for nominee & nominator.
- Divider 1: NACTA Graduate Student Award Checklist.
- Divider 2: Nominator's letter addressed to Chair of the Teacher Recognition Committee.
- Divider 3: Evidence of a current Graduate Student Membership (Statement), or indication of the date the membership application and check were sent to the NACTA Secretary/Treasurer. MAKE SURE YOU ADDRESS THIS!
- Divider 4: Teaching Philosophy (Not to exceed 5 typewritten double-spaced pages).
- Divider 5: Course Evaluations (Suggested that you submit 6-8 forms from current or former students). See main page for forms**
- Divider 6: Statement of support from faculty member(s) directly involved in nominee's teaching activities.
- Divider 7: Nominee must submit a complete description of their specific teaching involvement.
- Divider 8: Nominee must provide a self-evaluation of their teaching.
- Divider 9: Administrative Officer Evaluations (Minimum of one, but not more than three)
- Divider 10: Peer Evaluations (Minimum of one, but not more than three). See main page for form**
- Divider 11: Statement from nominee regarding their involvement in teaching outside of the classroom (clubs, review sessions, committees, presentations, etc.).
- Divider 12: Nominee's Resume/Vita
